2. Not a Medical Emergency Service

This website and our telehealth services are NOT intended for medical emergencies. If you are experiencing a psychiatric or medical emergency, please call 911 immediately or go to your nearest emergency room. 3. Services and Appointments

InnerRise Psychiatric Services & Wellness PLLC provides outpatient psychiatric care, therapy, medication management, and related wellness services to residents of Massachusetts. By scheduling an appointment, you agree to:

• Provide accurate and complete information about yourself and your health history
• Notify us promptly if you need to cancel or reschedule an appointment
• Review and sign all required intake, consent, and treatment forms prior to your first appointment
• Comply with our cancellation and no-show policy, which will be provided to you upon scheduling

4. Telehealth Services

InnerRise offers telehealth appointments via secure video and phone platforms for patients located in Massachusetts. By participating in telehealth services, you acknowledge that:

• You are located within the state of Massachusetts at the time of the appointment
• You have a private and secure location for your session
• You consent to telehealth as a mode of care delivery
• Technical failures may occasionally disrupt sessions, and in such cases, we will work with you to reschedule or continue your appointment

5. Fees, Insurance, and Payment

We accept many major insurance plans, including Medicare and Medicaid. You are responsible for understanding your insurance benefits, including co-pays, deductibles, and coverage limitations. By using our services, you agree to:

• Provide accurate insurance information and notify us of any changes
• Pay any amounts not covered by insurance, including co-pays and deductibles, at the time of service
• Be responsible for fees related to services deemed non-covered by your insurer

A current list of accepted insurance plans is available on our website or by contacting our office. Fees for self-pay patients will be discussed prior to or at the time of your first appointment.

6. Cancellation Policy

We require at least 24 hours’ notice for cancellations or rescheduling. Appointments cancelled with less than 24 hours’ notice or missed without notice (“no-shows”) may be subject to a cancellation fee. This fee is not billable to insurance and is the patient’s responsibility. Details of our specific cancellation fee schedule will be provided during the intake process.
